Services We Provide

 
 

Routine Care

Routine care is the basic everyday care that a patient will need while on our service. This includes nursing visits, medication management, personal care, social services, Chaplain services, bereavement services, and volunteer services. The extent of care needed may vary from patient to patient and increase or decrease in frequency as the care plan is adjusted. Almost all care is provided on an appointment based schedule. This may range from weekly visits all the way up to daily visits. This type of care is provided where ever the patient resides.


Continuous care

When a patient’s condition requires advanced pain management and symptom control, hourly nursing can be provided from 8 to 24 hours a day until a period in which the patient can feel comfortable without the continuous nursing care. This type of care is meant to give more focus to patients who are in an immediate crisis to help control their pain and alleviate any acute medical symptoms that are causing the discomfort. Along with an RN, a certified nursing assistant might also be present during this time. Continuous care is only provided for brief periods as necessary to maintain a patients comfort at home instead of them going to the hospital for treatment.

Inpatient care

During periods where a patient’s condition requires a level of care that cannot be provided in the home, a patient may be transported to a Medicare certified facility that is contracted through Hospice Services of GA. This type of care is normally for short periods of time and used mainly for crisis intervention. Many times this service is provided when the available tools for pain management are not available in the home, but are at a skilled-nursing facility. The facility must first be approved by Hospice Services of GA and we will coordinate with the patient and the family on means of transportation.


Respite Care

Caring for a terminally ill loved one can be stressful. Many times this task is asked of family members with little to no warning. While we all want to give our all during this challenging time, it’s important to care for ourselves as well. This is why we offer respite care. For a short period of time, a patient can be moved out of the home into a skilled nursing facility to give caregivers a chance to take a break and relax. This short break is often times what family members and caregivers need to continue the amazing work of caring for their loved one. All respite services are coordinated by a case manager with facilities approved by the hospice. During this time of respite, care is still provided to the patient by the hospice staff.
