Our vision is to be the hospice of choice for those facing a life limiting illness. We want to begin our relationship by asking patients and their families: “What do you want?” Then we can discover what their priorities are and tailor their care to support the patients individual goals and wishes. We strive to be the leader in all areas of palliative medicine, social services, spiritual counseling, grief support, along with pain and symptom management.

Hospice was founded by volunteers. As such, volunteers are a core service that we provide. Our company is proud to be able to offer a wide range of volunteer services to patients that are enrolled in our service.

Our Services

Hospice services differ depending on the level of care a patient requires. Most commonly, patients receive routine care in home. Services are also provided in personal care homes, skilled nursing facilities, assisted living centers, and est. We address all needs from medications, personal care, acute care, social work, spiritual care, and bereavement support. Care plans are adjusted to fit you and your family’s needs as the patient’s condition changes.

 

Skilled Nursing Care

Patients have access to a full range of nursing care. Our team of RN’s work directly with your hospice doctor to plan and provide care as needed. Nurses handle medication, changes in condition, and help facilitate the plan of care. Certified nursing assistants can help with personal care and hygiene regiments.

spiritual care

No matter if you are religious or not, a hospice Chaplain can be an asset to your family as you face this difficult time. Their role is to support your family spiritually. The Chaplain will work with your church on your behalf to help fulfill any religious request. Outside of religion, they also offer counseling during and after hospice.

Social work

Hospice is more than just medical care. The social workers that we employ help with many issues that might arise outside of the realm of medicine. They can act as a guide to help make decisions, coordinate with funeral homes, assist with insurance issues, and ensure the overall social well being of the patient and the family.

Bereavement care

Everyone experiences grief differently. When loved ones die, it can deeply affect someone’s emotional state. One of the benefits provided for those enrolled in hospice is bereavement counseling. After a patient has passed on, family members are eligible for free counseling. Sometimes it helps to talk and we are here for you.
